A Just Culture
A

s 2016 comes to a close and we reflect on all
the accomplishments of the aviation industry
during the past year, we must address the
safety systems that have been put in place by the various
authorities. The first observation is that they are a
resounding failure.
They fail, not because they fail to enhance safety, as
engaging in the conversation will do that. Rather, they
fail to address the industry as a whole. The safety of the
aviation industry begins with the head of the respective
aviation authorities and moves throughout the entire
industry to the lowest support person within individual
companies. That is a complete system for safety.
Unfortunately, every attempt at implementing system
safety insists on separating the governmental industry
from the private industry. This is highlighted in the
Safety Management Manual (Doc. 9859) published by
the International Civil Aviation Authority that states:
A clear understanding of the relationship between
an SSP and an SMS is essential for concerted
safety management action within states. This
relationship can be expressed in the simplest terms
as follows: States are responsible for developing
and establishing an SSP; service providers are
responsible for developing and establishing an
SMS. This is a very important point: States are
not expected to develop an SMS; rather the SSP
fulfils the equivalent role. Nevertheless, states

are responsible, as part of the activities of their
SSP, to accept and oversee the development,
Safety Management Manual implementation and
operational performance of the service provider's
SMS. In overseeing the safety performance of a
service provider's SMS, the notion of acceptable
level of safety of an SSP, discussed in 6.4, plays a
fundamental role in the relationship between an SSP
and an SMS.

You cannot empower employees
if there is an underlying fear of
retaliation and retribution.
Unfortunately, this is the equivalent of the corporate
office telling each division to step up and implement a
safety program independent of the chief executive. The
authorities' safety program and the industry's safety
program must be intertwined for the entire industry to
benefit. The starting point, in my humble opinion, is
with the establishment and fostering of a "Just Culture."
Eurocontrol defines Just Culture as "a culture in
which front-line operators and others are not punished
for actions, omissions or decisions taken by them which
are commensurate with their experience and training,
