a direct impact on those 12 common traits that lead to errors,
incidents and occasional accidents.
Think there is no parallel? Let's take something simple like
distraction. Ever seen someone driving their car and using a
mobile phone at the same time? Maybe a little nagging about
texting and driving is a good thing. We certainly see the
constant flood of public service announcements on television
commercials or every few miles on highway billboards. Maybe
you are the nagger with your spouse or children about texting
and driving. Most states have laws mandating hands-free
phone use, and yet we still have direct access to our phones. In
addition, we know, often from our own experience, that texting
or talking and driving can be distracting. How many times
have you missed a turn while talking on the phone?
According to the U.S. Centers for Disease Control and
Prevention, each day in the United States, "over eight people
are killed and 1,161 injured in crashes that are reported to
involve a distracted driver." It also notes that "distracted
driving activities include things like using a cellphone, texting
and eating. Using in-vehicle technologies (such as navigation
systems) can also be sources of distraction. While any of these
distractions can endanger the driver and others, texting while
driving is especially dangerous because it combines all three
types of distraction."
As a motorcycle rider, distracted driving is near and dear to
my heart. Sitting high in the saddle and being able to see what
other drivers are doing is downright scary. I have seen people
commuting into the city while reading newspapers, applying
makeup, eating and drinking, and of course, texting and talking
on mobile phones. But this isn't about distracted driving; this
is about communication. What do you do with the nagging?
The other modern tool in our business toolbox is safety
risk management, an element of SMS. Remember, a principle
element of SMS is employee reporting and then doing
something with that reporting (investigate, evaluate and
manage the potential risk). When you dig into the SRM aspects
of SMS, it is not only the employee reporting but also the
employer, the customer, or the regulator. The comments are all
treated the same: Investigate the report; evaluate the comment;
perform an analysis of the noted deficiency; plan a corrective
action; and complete the corrective action and perform a
follow-up. We identify the risk, and then we manage the risk.
Through communication, we are made aware of a possible
error. Since we know the dirty dozen are the most common
causes of errors, we can start with those. Was my error caused
by a lack of communication? Was I complacent or distracted?
Did I lack knowledge, teamwork or resources? Was my
decision affected by fatigue, pressure or stress? If I suspected
the potential error, could I have been more assertive? Have I
been doing things this way so long they are now a norm?

The dirty dozen is a quick reference to why an error
occurred. But we also must ask the following question: Why
did that item lead to an error, or was it a contributor to the
error?
Why did I miss my exit off the highway? Well, I was on the
phone. "Maybe you shouldn't do that!" Yeah, yeah, I've heard
that nagging so many times, it's just noise now. Not only was I
distracted, but now I am complacent even when being nagged
about it. Sound familiar?
Or maybe it's just the norm today. One newspaper headline
this spring read as follows: "Trucking employee claims he was
fired for not texting and driving 'like everyone else.'" Yes, you
read that correctly; he was fired for not texting and driving.
I don't know anything about this event, but the headline
certainly gets your attention. I'm not sure I would want to be
the labeled employer, but it says a lot about the company's
norms.
What you do, not what you say, is your norm. What your
employees do after work and on the weekends when not being
supervised also is the norm. If an error is rooted in the norm,
maybe the norm needs to change. Maybe our complacency just
hasn't caught up with us - yet.
I like to look at the dirty dozen as four groups. We have
already noted the norm, which is a group all by itself.
Communication, complacency and distraction are bundled
together since these tend to be personal traits. Internal and
external factors may lead to fatigue, pressure or stress.
Finally, the missing elements: a lack of knowledge, teamwork,
resources, assertiveness or awareness, which provides me with
the opportunity to add something. It may be physical, such as
a lack of tools or test equipment. Or it may be mental, such as
a lack of teamwork or assertiveness. Either way, it identifies an
area for improvement.
We talk about safety management systems as if it is
something being forced upon our businesses by the respective
regulatory authorities. But as you can see, the elements
of employee (friend and family) reporting, safety risk
management and safety assurance can be used in everyday life.
And I would suggest, either consciously or subconsciously, the
elements of SMS are used in everyday life.
As I noted previously, we lost a passionate aviator earlier
this year far too soon. I lost a friend. He was quick to criticize
if he disagreed with you. But throughout the years, his
recommendations and mentoring were much more rewarding
than the occasional criticism. "Sadly, (his) Misguided Strategy"
allowed life's errors to take him from us long before we
were ready. Maybe he lacked knowledge or didn't act on his
awareness. Maybe I wasn't assertive enough or didn't nag him
enough. Maybe we just accepted the norm.
