Maryland Bridge Pontic Retainer for
Finished Cases with Missing Laterals

On occasion, patients who present with missing
laterals will undergo orthodontic treatment with
plans to maintain the edentulous spaces for
future restorations. This, however, presents an
esthetic concern for the patient until a permanent restoration can be delivered. In such cases,
a Maryland bridge pontic retainer can serve as a
temporary solution after orthodontic treatment
and until a final restoration can be placed.
Additionally, this method prevents adjacent
teeth from tipping into the edentulous sites,
thereby preserving mesiodistal space and maintaining root parallelism achieved by orthodontic
treatment.

to fabricate the retainer. At this appointment,
shade selection for the pontic occurs. The lab
then fabricates the appliance using .036 stainless
steel wire and provides a transfer tray for bonding the retainer with composite pads.

One visit prior to debonding a patient, an impression or an iTero scan is taken and sent to the lab

The patient returns for the debond appointment
and the retainer is bonded onto the teeth. The
composite pads on the retainer are sandblasted
and the teeth are prepared using etch, OptiBond
Solo, and then flowable composite on the pads.
The transfer tray is then placed and the retainer
is light cured in place; the occlusion is checked
and interproximal contacts are flossed. Impressions for Essix retainers then follow, which the
patient will wear at night time throughout
retention. 
