BH: I'm sure in that type of environment you had
some interesting experiences.
These patients were waiting when
we arrived. This is at a remote
mountain village near Los Anices,
Honduras.
DE: In Chiapas, Mexico, I spent three weeks treating the Zapatista rebels in an abandoned hospital. The rebels all wear masks in their constant
conflict with the Mexican army. The Zapatistas
became my friends; I never felt that I was in any
danger. On another trip, in remote Honduras, I
was thrilled to help deliver a baby - with a full
professor of obstetrics standing at my side.
BH: What procedures are offered in your clinics?
Is your work primarily exodontia?
DE: Yes, my individual clinics are limited to dentoalveolar surgery, such as extractions, pre-prosthetic
surgery, and an occasional oral-antral fistula
repair or biopsy. Fascial space infections are
treated aggressively; in the developing world,
odontogenic infections can be fatal. In the Philippines, addiction to methamphetamine ("shabu")
is epidemic. Many meth addicts require fullmouth extractions. The clinic pace is pretty fast.
I try to remove an average of 15 teeth per hour.
BH: Do you ever have emergencies? Have you
ever called 911?
DE: Emergencies are rare, but over the last
decade I have administered every emergency
drug in my kit. I have injected epinephrine twice:
once for an infant with a severe, prolonged
asthma attack and again for an adult with a bee
sting. When indicated, epinephrine is magic.
In remote areas, there is no emergency service. In
the Philippines, we once called 911. That was two
years ago; we are still waiting for them to show up.
At a Haitian refugee camp,
Dominican Republic, where the
village has extreme poverty.
Unemployment is 100%, and
residents live off the land, often
eating plant roots and sugar cane.
Every child has head lice and all
clothes are donated.
Zapatista rebels are independent,
proud, and poor. They all wear
masks, even the children. This is in
Chiapas, Mexico.
