Case Report Special Section RETENTION Case J.K. 10 years How would you treat this malocclusion? Pre-Treatment A 10-year-old male patient presents in good health with a non-contributory medical history. The child's request is, "I want braces; I don't like my overbite." His mother's chief concern is, "His front teeth are getting worse; he has speech problems." He is in the mixed dentition stage, with Class I molar on the right and end-on Class II on the left. There are 8 mm of overjet and 4 mm of overbite. His mandibular incisors are proclined. His gingival health is very good. No severe crowding is anticipated. Extraoral and Intraoral Findings The patient has a long but symmetrical lower face, with lips parted at rest. His maxillary incisors are ahead of the glabella and his chin appears retruded. With lips closed, there is considerable strain and they are quite protrusive. He displays 90% of his maxillary teeth while smiling, with no gingival display. Radiographic Findings His maxilla is slightly protrusive while his mandible is retrognathic and hyperdivergent. Treatment Objectives Treatment objectives for this patient are to align his maxillary anteriors, maintain the anteroposterior and vertical position of his maxilla, monitor for mandibular growth, and consider the possibility that orthognathic surgery might be needed in the future.
