innovative palliative care program
Baylor Scott & White Health became the
first large US health system to implement
the Serious Illness Conversation and Care
Planning Program developed by Ariadne
Labs, a joint center of Brigham and Women's
Hospital and Harvard T.H. Chan School of
Public Health. The program is drawn from
best practices in palliative and serious illness
care and facilitates compassionate, patientcentered discussions about serious illness
between clinicians, patients, and their families.
Because of the significant shortage of palliative
care providers, the program aims to train
family practitioners, internists, cardiologists,
oncologists, geriatricians, and others who care
for seriously ill patients to have more frequent,
better, and earlier conversations fostering more
patient-centered care than might otherwise
occur. Scott & White Medical Center - Temple
began implementing the program in October
2016 starting with oncologists, cardiologists,
and geriatricians. Baylor Scott & White hospitals
in North Texas began in March 2017. The
program provides a system for identifying
appropriate patients, extensive training courses
and coaching for clinicians, the Serious Illness
Conversation Guide for clinicians and patients,
patient preparation materials, suggestions to
help patients discuss care preferences with
family, and a system for documenting patient
goals and priorities in Baylor Scott & White's
electronic medical record system.

of Clinical Ethics and Palliative Care for Baylor
Scott & White Health. "The Serious Illness
Conversation and Care Planning Program
allows Baylor Scott & White to take a population
health approach to give more patients and
families facing serious illness the opportunity to
make informed choices that reflect their values,
reduce suffering, enhance family well-being,
and improve their quality of life, even if a cure
might not be possible."

AccentCare to form joint venture
Baylor Scott & White Health and AccentCare,
Inc., a nationwide leader in post-acute health
care, have signed an agreement of intent to
establish a new home health joint venture. The
new joint venture will combine four existing
agencies from Baylor Scott & White and six
from AccentCare, with AccentCare managing
all operations. "AccentCare shares our
commitment to innovative and compassionate
patient care and brings significant home care
experience to our collaboration," said James
H. Hinton, president and CEO of Baylor Scott
& White Health. "As health care delivery
continues to evolve, this joint venture helps
to ensure that high-quality integrated care is
available to our patients and their families in
their homes."

membership-based medical practice
and primary care clinics in Tom
Thumb stores
Baylor Scott & White Health announced
Baylor Scott & White Direct Care-a
membership-based medical practice located
in the Park Cities neighborhood of Dallas. For
an annual fee of $200, members can expect
access to quality physicians and advanced
practice providers 7 days a week. The practice
is operated by the health care system's affiliate,
HealthTexas Provider Network, a multispecialty
medical group that employs more than 1300
providers practicing in the North Texas area.
"The unique and innovative model takes patient
care to the next level-offering a modern,
relaxed atmosphere with the latest technology,
as well as same-day access to providers and
extended hours," said F. David Winter, MD,
president and chairman, HealthTexas Provider
Network.
In addition, five primary care clinics opened
in Tom Thumb stores throughout North Texas in
April and May 2017. These clinics, located in
Oak Cliff, Arlington, Fort Worth, and Mansfield,
are also operated by HealthTexas Provider
Network. "Through the new collaboration
with Tom Thumb, we're able to offer quality,
affordable basic primary care treatment in
convenient locations," said Dr. Winter. "Each
clinic will be open 7 days a week, making routine
primary care services even more accessible."
